Privacy Policy
The privacy of patient information is paramount within the Treat Unique practice and as such, we will go to great lengths to handle your information according to the relevant Privacy Acts currently held and updated (Privacy Act 1988 (Cth), the Privacy Amendment (Enhancing Privacy Protection) Act 2012, the Australian Privacy Principles and relevant State and Territory privacy legislation (referred to as privacy legislation).
What information will we record?
We will record all information that is relevant to your care – this may include, but is not exhaustive to, all aspects of the medical history, psychosocial history, referrals, reviews, and procedures. Some information will be recorded on our medical information servers to identify you (i.e. name, age, date of birth, address, contact numbers etc). We will record all aspects of your care on our electronic medical record which can include e-mails, telephone conversations, text message correspondence and imaging and pathology faxed results. We may also record, where relevant, next of kin information and contact details in the event of an emergency.
Where is the record collected?
The medical information will be stored electronically in a password protected manner on our computer systems or, in some instances, in hard-files, locked at the medical practice. At al times this information shall remain private and can only be accessed by health care and allied health providers involved in your care along with secretarial staff. There are certain circumstances where, by law, we are required to share your information with third parties (i.e police, medicare, hospitals, courts).
Access to your own medical record
Occasionally patients would like a copy of the medical records. Should you require this we would request that you put in a formal request in writing to our practice and we will endeavor to provide you with this in a timely fashion.
Complaints
Complaints are never nice but an inevitable part of life. Please if you are dissatisfied with any aspect of our service, record keeping, privacy policy etc we encourage you to raise this with us at the time. If it suits you, please send us an email or letter to outline your complaint in detail. We will take all complaints seriously and try to work with you to fix the problem at hand.
